HealthWest Partnership and Inner North West PCP (INWPCP) have partnered with the National Disability Service (NDS) to bring you the Western Melbourne Cross-Sector Community of Practice, an initiative of the Ageing, Disability, Mental Health Collaborative Panel. The purpose is to bring together different sectors to collaborate and exchange good practice to achieve better consumer-directed care outcomes. Using a thematic approach, the network aims to support service providers to successfully navigate multiple service reforms.
We will hold 2 events in the first half of 2019:

These forums follow on from three successful forums held in 2017 and two in 2018.
Participation is open to any clinician/worker, team leader or manager representing services that are interested in progressing a consumer-directed care approach within their organisations across the west.
These forums will assist participants by providing examples of consumer-directed care and innovation, while offering a great opportunity for networking and collaboration. See the Terms of Reference: Western Cross-Sector Community of Practice
The National Disability Service held a combined forum with the four Communities of Practice from across Victoria for a combined forum ‘Collaboration in a competitive market’ held on 24th July 2018.
The forum explored how we can collaborate in the emerging competitive market being created by the reforms in Aged Care, Disability and Mental Health. The forum also enabled sharing of key learnings from the Community of Practice project which is led by NDS as part of Collaborative Panel initiatives.
