Western Cross Sector Community of Practice

Overview

HealthWest Partnership and Inner North West PCP (INWPCP) have partnered with the National Disability Service (NDS) to bring you the Western Melbourne Cross-Sector Community of Practice, an initiative of the Ageing, Disability, Mental Health Collaborative Panel. The purpose is to bring together different sectors to collaborate and exchange good practice to achieve better consumer-directed care outcomes. Using a thematic approach, the network aims to support service providers to successfully navigate multiple service reforms.

 

Community of Practice Forums

Between January 2017 and June 2019, we held 7 forums on the following topics:

  • Assisting clients to navigate multiple service settings (March 2017)
  • Promoting self-advocacy in the context of transition (August 2017)
  • Practical workforce solutions to cultural change: cultivating a values-based commercial culture (November 2017)
  • What does the future workforce look like in the reform environment? (March 2018)
  • Integrating consumer choice and control principles into service design and delivery (November 2018)
  • Consumer choice and control (March 2019)
  • What does health literacy have to do with access and inclusion? (May 2019)

These forums were coordinated by a Steering Committee. See the Terms of Reference: Western Cross-Sector Community of Practice Steering Committee

These forums were positively received, providing participants with opportunities to hear consumer perspectives, exchange good practice and build cross-sector relationships.

Community Cross-sector Forum

The National Disability Service held a combined forum with the four Communities of Practice from across Victoria for a combined forum ‘Collaboration in a competitive market’ held on 24th July 2018.

The forum explored how we can collaborate in the emerging competitive market being created by the reforms in Aged Care, Disability and Mental Health. The forum also enabled sharing of key learnings from the Community of Practice project which is led by NDS as part of Collaborative Panel initiatives.
