Western Cross Sector Community of Practice

Overview

HealthWest Partnership and Inner North West PCP (INWPCP) have partnered with the National Disability Service (NDS) to bring you the Western Melbourne Cross-Sector Community of Practice, an initiative of the Ageing, Disability, Mental Health Collaborative Panel. The purpose is to bring together different sectors to collaborate and exchange good practice to achieve better consumer-directed care outcomes. Using a thematic approach, the network aims to support service providers to successfully navigate multiple service reforms.

 

Community of Practice Forums

We will hold 3 forums in 2018:

  • What does the future workforce look like in the reform environment? – Tuesday 20th March 2018, 9:30am – 12:30pm
  • Engaging Hard to Reach Groups in Consumer Directed Care – Tuesday 14th August 2018, 9:30am – 12:30pm
  • Co-design – Monday 12th November 2018, 9:30am – 12:30pm.

Community Cross-sector Forum

The National Disability Service will also bring four Communities of Practice from across Victoria together for a combined forum:

  • Collaboration in a competitive market – Tuesday 24th July 2018, 1:30pm – 4:30pm (registrations from 1:15pm). Registration for this free event is through Eventbrite.

This forum will explore how we can collaborate in the emerging competitive market being created by the reforms in Aged Care, Disability and Mental Health. The forum will also enable sharing of key learnings from the Community of Practice project which is led by NDS as part of Collaborative Panel initiatives.  There will be opportunity for networking.

Who Can Attend?

Participation is open to any clinician/worker, team leader or manager representing services that are interested in progressing a consumer-directed care approach within their organisations across the west.

Why Come Along?
